• Declaration of Intent Regarding Preneed Funeral Arrangements: Ensures a decedent’s written wishes are clearly recorded and honored under California Health & Safety Code 7100.1

• Identification and Authorization Form: Documents the identification process and the individual authorized to control disposition, providing indemnification and compliance support

• Worksheet for Determining Legal Next of Kin: Helps funeral professionals evaluate next-of-kin relationships in alignment with California’s hierarchy of rights and responsibilities
