Applying for Membership

Applying for CFDA Membership consists of two parts - submission of an Application and Code of Ethics.  Please print out both parts, fill out completely, and fax to: (916) 444-7462 or mail to:

One Capitol Mall, Suite 320
Sacramento, CA 95814

Both the Application and CFDA Code of Ethics must be signed and submitted together with the appropriate application fees.

Membership Application Process
Upon receiving your completed application and Code of Ethics, the CFDA Membership Committee will review your request for member status and grant your firm tentative member status. Once approved, your funeral home will forward its dues to CFDA, and in return, will receive a Compliance Resource Manual, Membership Plaque, Cal-OSHA Compliance Program, and Membership Directory, in addition to many other benefits of membership.
